Childcare Reimbursement Form

Hey there! We understand that the cost of childcare can at times be a barrier to attending an event. For certain events, The Co offers reimbursement for the cost of childcare to parents who need the assistance in order to participate. In those cases, please plan to pay your sitter on the day they work. Then use this form to request personal reimbursement for that payment (up to $15/hour) after the event.
